which includes the Code of Conduct, the program scope and

- Submit
credit worthiness information and send a $280.00 check made
payable to Columbia Gas of Virginia to:
Columbia Gas of Virginia
c/o Gas Transportation
200 Civic Center Drive
Columbus, OH 43215.
- Once
approved, submit a completed Choice Aggregation Agreement.
Columbia Gas of Virginia will notify you of approval/disapproval
to participate as a Choice gas supplier. You may also need a Marketer
Rate Statement form.
